To improve patient experience from 1st November 2023 we are implementing a new digital referral system for new pregnancy for patients or nominated person to self-refer to SBUHB Maternity services via a QR code or by using the following link to register pregnancy on Swansea Bay website.
Go to the Swansea Bay website to self refer or use the QR code on the poster below to access the form.
Complete the online form on the Swansea Bay website and you will receive an email to confirm your form has been safely received.
Within 1 to 2 weeks, a community midwife will contact you to arrange an appointment
