Form Med 3 “Statement of Fitness for Work”
You may need to give this form to your employer or to the Job Centre Plus, DWP (Dept Work and Pensions) if you are unable to work due to sickness for more than 7 days. Please note that doctor will only consider issuing a Med 3 after the seventh day of sickness.
Patients who have been sick for more than 7 days should arrange an appointment with Doctor to obtain a Med 3. There is no charge made for the provision of Med 3s to be given to your employer or to the Job Centre Plus. Should a Private Sick Note be requested (eg for Insurance purposes) then a fee of £15.00 will be payable.
Form SC2 “Employee’s Statement of Sickness”
If you are an employee, your employer may ask you to complete their own self-certificate form or Form SC2 “Employer’s Statement of Sickness” to cover you for the first seven days of incapacity.
